Job Description:

A company is looking for a Registered Nurse - RN - Case Management - Remote.

Key Responsibilities

Assess, plan, implement, and coordinate case management activities for members
Conduct evaluations of members' needs using clinical tools and data review
Utilize motivational interviewing to engage members and assess their health status

Required Qualifications

Must be a Registered Nurse with an unrestricted nursing license and multi-state/compact privileges
3+ years of clinical/nursing practice experience
1+ year of medical case management, care coordination, or discharge planning experience
COVID vaccination is required
Associate's degree in nursing required; BSN preferred
