Job Description:
A company is looking for a Case Manager Social Work to
coordinate psychosocial community services for patients.
Key Responsibilities
Assist in the assessment, planning, and implementation of
psychosocial programs
Counsel patients and communicate their psychosocial needs to the
care team
Facilitate patient access to medical providers and community
services
Required Qualifications
Bachelor of Science or Arts in Social Work, Counseling, Psychology,
or a related field
Licensed Social Worker in New Jersey
Experience in caring for chronic disease patients
Experience with local medical and social support systems
Experience with Electronic Medical Records and Microsoft Excel
