Job Description:
A company is looking for a Care Manager Transition of Care
(BH).
Key Responsibilities
Evaluate member needs through post-discharge assessments and
medication reconciliation
Develop care/service plans and collaborate with healthcare teams to
support member transitions
Facilitate education and resources to promote wellness and improve
overall quality of care
Required Qualifications
Master's degree in Behavioral Health or Social Work, or a degree
from an accredited School of Nursing
2 - 4 years of related experience in care management
Valid license such as LISW, LCSW, LMSW, LMFT, LMHC, LPC, or RN
Ability to work remotely within the Central time zone
Compliance with state, federal, and third-party payer
regulations
