Job Description:
A company is looking for a full-time Virtual Nurse Practitioner
to join their high-risk home-based care team supporting patients in
the Colorado area.
Key Responsibilities
Manage patients with complex conditions through tele-assist home
visits and remote interventions
Conduct thorough initial and follow-up visits focusing on
geriatrics and chronic condition management
Collaborate with multidisciplinary care teams to enhance patient
care and reduce preventable hospitalizations
Required Qualifications
5+ years of experience as a Nurse Practitioner, with at least 2
years in home-based or virtual complex care
Current licensure in Colorado and willingness to obtain additional
state licenses if needed
Experience with home-based care and value-based care principles
Ability to work in MST time zone and adapt to a fast-growing
organization
Strong technology skills and proficiency in telehealth
platforms
