Job Description:
A company is looking for a Health Plans Associate Behavioral
Health Case Manager.
Key Responsibilities
Manage individual members across the health care continuum to
achieve optimal outcomes
Lead process improvement activities for patient populations to
enhance clinical and operational outcomes
Establish collaborative relationships with providers and
communicate necessary information for care coordination
Required Qualifications
Bachelor's degree in social work or a related field, or equivalent
experience
Five years of experience in care management or a related area
Working knowledge of hospital and community resources
Ability to work effectively in an interdisciplinary team format
Demonstrated critical thinking and time management skills
