Job Description:
A company is looking for a Nurse Practitioner to perform in-home
health assessments on Medicare Advantage Members in New York.
Key Responsibilities
Conduct in-home health assessments for Medicare Advantage
members
Cover designated counties in New York, with flexible scheduling
options
Complete assessments ranging from 45-90 minutes in length during
operational hours
Required Qualifications
Minimum 6 months of experience as a Nurse Practitioner
Active NP license and national certification (ANCC or AANP)
All board certifications accepted except PMHNP or WMNP
Experience in gerontology, cardiology, internal medicine, or
endocrinology is a plus
Home health care or home visit experience is a plus
