*Job Summary:* The Clinical Supervisor - Home Health (RN) plays a crucial role in overseeing clinical operations, ensuring the delivery of high-quality home health services. Reporting to the Director of Clinical Services or Executive Director, this position is responsible for managing assigned staff and ensuring compliance with policies, procedures, and time management standards. The Clinical Supervisor develops, implements, and evaluates goals and objectives that align with agency standards and contribute to overall organizational success. Additionally, this role includes patient, caregiver, and family counseling, as well as effective communication with physicians and healthcare providers to coordinate patient care. The Clinical Supervisor may also conduct periodic field visits to administer skilled care to patients as needed.

*Essential Responsibilities:*
* Direct and manage clinical operations in the assigned branch office, ensuring quality and efficiency in home health services.
* Establish, implement, and evaluate goals and objectives that promote high standards of patient care.
* Ensure compliance with agency policies, Medicare regulations, and accreditation criteria.
* Supervise, mentor, and support nursing and clinical staff to enhance performance and patient outcomes.
* Participate in patient, caregiver, and family education and counseling.
* Communicate with physicians and healthcare practitioners regarding patient care plans.
* Assist in the development, evaluation, and revision of patient care plans in collaboration with patients and caregivers.
* Participate in ongoing interdisciplinary assessments and quality assurance initiatives (QAPI program).
* Perform periodic skilled nursing visits as needed.
* Serve as Administrator, Alternate Administrator, Director of Nursing, or Alternate Director of Nursing, as required, in compliance with Agency for Health Care Administration (AHCA) regulations.
* Participate in on-call rotations, at least once per month or as needed.

*Qualifications & Requirements:*
* Active Registered Nurse (RN) license in the state of practice.
* Minimum of three (3) years of nursing experience; home health experience preferred.
* CPR certification (American Heart Association or American Red Cross).
* Extensive nursing experience with an emphasis on home health care preferred.
* Clinical management experience, with the ability to supervise, mentor, and direct staff.
* Strong knowledge of Medicare regulations, accreditation criteria, and home health standards.
* Ability to work independently while demonstrating flexibility, initiative, and problem-solving skills.
* Excellent written and verbal communication skills with strong interpersonal abilities.
* Proven ability to collaborate with diverse teams and achieve common goals.
* Strong critical thinking, assessment, and customer service skills.
* Valid driver's license, auto liability insurance, and reliable transportation.
* Willingness to travel as needed.

*Education Requirements:*
* High school diploma or GED required.
* Graduate of an accredited school of nursing.

*Working Conditions & Physical Requirements:*
* Work is conducted in both the agency office and patient homes, requiring occasional travel.
* Moderate physical effort, including kneeling, squatting, reaching, twisting, climbing, and walking.
* Must be able to assist in lifting and transferring patients up to 100 pounds.
* Functional sight and hearing or appropriate adaptive devices required.
* Must provide evidence of an annual TB test and other state-mandated health screenings.
* Exposure to various environmental conditions, including odors, bodily fluids, and hazardous materials.

*Work Location:* On-Site: 13100 Northwest Fwy, Suite #400 A, Houston, Texas 77040

*Work Schedule:* Monday to Friday, 8:30 AM - 5:00 PM
