Job Description:

A company is looking for a Clinical Pharmacist (MTM Support).
Key Responsibilities

Evaluate medication profiles to optimize drug therapy and minimize adverse reactions
Recommend alternative medications, dosing modifications, or discontinuation of medications
Conduct telephonic comprehensive medication reviews with members and collaborate with physicians as needed

Required Qualifications

Bachelor's or Doctorate of Pharmacy degree
Minimum one year of pharmacy residency training or equivalent experience recommended
1-2 years of experience in a pharmacy or related health care environment
Current, valid & unrestricted state Pharmacy license required
May need to obtain additional Pharmacy licensure in multiple states within six months of employment
